Tibial sesamoiditis is an inflammatory condition affecting the sesamoid bones, two small bones located beneath the big toe joint. These bones act like pulleys for tendons and help absorb weight-bearing pressure during walking and running. When the tibial sesamoid becomes irritated or inflamed, pain develops at the ball of the foot, especially under the big toe.
This condition is often caused by repetitive stress, overuse, improper footwear, high-impact activities, or abnormal foot mechanics. Runners, dancers, athletes, and individuals who spend long hours on their feet are particularly at risk. Symptoms may include localized swelling, aching or sharp pain, and discomfort that worsens with activity.
Early diagnosis is important to prevent chronic inflammation or stress fractures of the sesamoid bones.
Sesamoiditis is a generalized term for a painful inflammatory condition associated with a sesamoid complex devoid of radiographic changes.
Physical examination can reveal pain with direct palpation or passive distal push on the sesamoid apparatus, passive bending of the hallux, and pain along the tendons heading toward the big toe.

• Big toe joint arthritis is also known as hallux rigidus and can form a very large toe joint spur.
• A turf toe is the hyperextension and bending of the big toe joint. This can cause ligament and tendon damage.
• Sesamoiditis of the big toe joint is the inflammation and possible fracture of the two bones underneath your big toe joint.

Highlighted are the fibular sesamoid on the right hand side, a tibial sesamoid on the left hand side.
The tibial sesamoid is the most likely injured sesamoid, because it is the one that we put most pressure on two.
On the other hand, the fibular sesamoid is most likely to squeeze out to the lateral side, or towards the fifth toe during the bunion dislocations.
