When Is Toenail Matrixectomy Needed?

Toenail matrixectomy is typically recommended when non-surgical treatments fail and the ingrown toenail continues to recur or becomes infected. It is also advised for patients with thickened, damaged, or deformed toenails that cause discomfort.

Our Surgical Approach

Our doctors perform a partial or full matrixectomy depending on the severity. The procedure involves numbing the toe, removing a portion (or all) of the nail, and applying a chemical or using a laser to destroy the nail matrix to prevent regrowth.

Benefits of Toenail Matrixectomy

  • Permanent solution for chronic ingrown toenails

  • Reduces risk of repeated infections

  • Quick, in-office procedure

  • Minimal downtime

  • High success rate

What to Expect – Before, During & After Surgery

Before: You’ll undergo an assessment and discuss any ongoing symptoms.
During: The area is numbed, and the nail or nail section is safely removed.
After: Most patients return to normal activities within a day or two, with some aftercare and bandaging.

Frequently Asked Questions About Toenail Matrixectomy

Is toenail matrixectomy painful?

How long does recovery take?

Recovery is quick—usually 24 to 48 hours. Some minor drainage or tenderness is expected.

Will the nail grow back?

Not if the matrix is completely removed. This is the goal of the procedure to prevent recurrence.

Can I walk after the procedure?

Yes, but you should avoid strenuous activity for a day or two. Loose-fitting shoes are recommended.

Is this covered by insurance?

Yes, most insurance plans cover toenail matrixectomy. We can help verify your coverage before treatment.

Products for Recovery After a Matrixectomy

These are the products we actually stock at our Howell and Bloomfield Hills clinics and hand to patients dealing with post-matrixectomy. Each one has clinical evidence behind it — and each one has a patient it is not right for. We have included that, too, because we want you to pick well. Links below support the research behind this site; prices and availability vary.

  • Epsom salt + povidone iodine soak — Daily 10-minute soak for the first 10 days post-procedure. Prevents the deep infection that is the main reason matrixectomies fail. Not ideal for: patients with diabetes or neuropathy — we manage your aftercare in-clinic instead. See what we stock.
  • Open-toe medical sandal — Required footwear for 2–3 weeks post-procedure to eliminate pressure on the treated nail bed. Not ideal for: active lifestyles during the first 2 weeks (scale back activity regardless of shoe). See what we stock.
  • Doctor Hoy’s Natural Pain Relief Gel — From post-op day 3 onward — apply around the nail fold for tenderness. Faster pain-free return to closed-toe shoes. Not ideal for: signs of infection (redness spreading, purulence) — call our office, do not self-treat. See what we stock.

Not sure which is right for your foot? We fit and demo these in clinic and can pair them with the right shoe. (810) 206-1402 · book online. Same-day appointments available.

Frequently Asked Questions

What is the difference between partial and total matrixectomy?

Partial matrixectomy removes one or both sides of the nail permanently — ideal for recurring ingrown toenails. Total matrixectomy removes the entire nail permanently — used for severely deformed, thickened, or fungal nails causing chronic problems. Both use phenol chemical to destroy nail matrix cells.

Will I have a nail after matrixectomy?

After partial matrixectomy, the central nail remains intact and looks nearly normal — only the offending edge is gone. After total matrixectomy, no nail grows back. Most patients with total matrixectomy report improved appearance and complete elimination of pain.

How quickly can I return to normal shoes after matrixectomy?

Most patients wear regular shoes within 3–5 days. There may be minor drainage for 1–2 weeks as the chemical area heals. Athletic activity is typically resumed within 1–2 weeks. The procedure has virtually no recovery impact on daily life.
