Pickleball is now the fastest-growing sport in the United States — and I’m seeing the injuries to prove it. Ankle sprains, knee pain, plantar fasciitis, and stress fractures are all showing up in my office from players who are playing on the wrong shoes.
Here’s what pickleball does to your feet that most sports don’t — and exactly which shoes to wear for it.
Why Pickleball Is Uniquely Demanding on Your Feet
Pickleball combines elements of tennis, badminton, and ping pong on a smaller court. That sounds low-impact. It isn’t — at least not for your feet and ankles. Here’s why:
- Rapid lateral movement — unlike running, which is mostly forward, pickleball requires constant side-to-side cuts. This creates lateral ankle stress that running shoes aren’t designed to handle.
- Quick stops and starts — decelerating suddenly from a sprint puts enormous force on the forefoot and plantar fascia
- Pivoting on the ball of the foot — common in pickleball serves and volleys, stresses the sesamoid bones and metatarsophalangeal joints
- Hard court surface — unlike grass or clay in tennis, most pickleball courts are hard concrete or asphalt, maximizing impact force
- Older player demographics — most pickleball players are 50+, meaning more arthritis, more osteoporosis risk, and less ankle ligament elasticity
The #1 Mistake Pickleball Players Make: Wearing Running Shoes
Running shoes are designed for linear, forward motion. The flexible, cushioned midsoles that make them excellent for running make them dangerous for lateral sports. When you plant and cut sideways in a running shoe, the flexible sole doesn’t provide the lateral wall support needed — your ankle can roll over the edge of the sole, causing a lateral ankle sprain.
I see this constantly. Patient comes in with an ankle sprain. They were wearing their favorite running shoes for pickleball. The shoe that protects them on a run fails them completely on the court.

Ankle Bracing: Should You Wear One for Pickleball?
For players with a history of ankle sprains or ankle instability, a lace-up ankle brace worn inside the court shoe provides an additional layer of protection. This is especially true on outdoor courts where the surface can be uneven.

Metatarsal stress fractures. Specifically, the 2nd and 3rd metatarsals in players over 60 who are playing 4-5 times per week on hard courts in running shoes or inadequate court shoes.
Stress fractures develop from repetitive cumulative stress — not a single impact but thousands of subthreshold forces that eventually exceed the bone’s remodeling capacity. Hard courts + inadequate footwear + high frequency play + older bone density = stress fracture risk.
If you have forefoot pain that worsens with activity and has been present for more than 2 weeks, get evaluated. A stress fracture that goes untreated becomes a complete fracture.
My Recommendations for New Pickleball Players
- Buy court-specific shoes before you play your first game — not after your first injury
