Toenail discoloration — whether yellow-brown thickening, black spots, white patches, or green streaks — is one of the most common reasons patients visit a podiatrist, and one of the most frequently misdiagnosed. The color of nail discoloration provides important diagnostic information, but accurate diagnosis requires clinical examination and sometimes laboratory testing, because many conditions can look similar. At Balance Foot & Ankle in Southeast Michigan, Dr. Tom Biernacki evaluates toenail discoloration systematically to ensure the right diagnosis and right treatment.

What Different Toenail Colors Mean

Yellow-brown thickening: the classic presentation of onychomycosis (toenail fungus) — dermatophyte infection causing yellow-brown discoloration, thickening, brittleness, and debris under the nail. However, nail psoriasis, chronic trauma in athletes (runner’s nail), and onychogryphosis (hypertrophic nail) look nearly identical. Fungal culture or PAS stain of nail clippings is required for confirmation before prescribing oral antifungals. White discoloration: superficial white onychomycosis (caused by Trichophyton mentagrophytes — powdery white on the nail surface); keratin granulation (chalky white patches from nail polish removal — the most common cause of white toenails in women); leukonychia (white spots from minor trauma to the nail matrix — “milk spots”); or total leukonychia (associated with systemic conditions). Black/dark discoloration: subungual hematoma (blood under the nail from trauma — most common cause of dark nails) is usually benign and grows out; however, subungual melanoma (the most dangerous form) must be excluded for any dark streak in the nail that doesn’t grow out, has irregular borders, or involves the nail fold (Hutchinson’s sign). Green discoloration: Pseudomonas aeruginosa bacterial infection (green-black) — occurs under lifted nails or in nail folds; requires topical or systemic antibiotic treatment. Yellow nail syndrome: yellow thickened slow-growing nails associated with lymphedema and pleural effusion — systemic condition.

Seek prompt evaluation for: any dark streak in the toenail that doesn’t correspond to a known injury, has irregular borders, involves the cuticle (Hutchinson’s sign — a red flag for subungual melanoma), or has been present for more than 3 months without moving distally. Also seek evaluation for: painful nail with spreading redness or warmth (infection); any toenail problem in a diabetic patient — nails can harbor fungal infections that create portal-of-entry wounds; and nail changes with systemic symptoms (joint pain, skin rash, nail pitting — may indicate psoriatic arthritis).

No — yellow toenails are most commonly fungal (onychomycosis) but can also be caused by nail psoriasis, chronic trauma, onychogryphosis, and yellow nail syndrome. Visual diagnosis alone has only a 50–60% accuracy for toenail fungus. Before starting oral antifungal treatment (which has potential liver and drug interaction effects), confirm the diagnosis with a fungal culture or nail clipping PAS stain. Treatment differs significantly by cause — oral antifungals treat fungal infection but won’t help psoriasis or traumatic nail changes.

Why is my toenail black?

A black toenail after known trauma (dropping something on the toe, repetitive shoe pressure from running) is almost always a subungual hematoma — blood trapped under the nail. Toenail fungus is confirmed by: fungal culture (nail clippings cultured in the laboratory — 3–4 weeks for results; identifies the specific organism); PAS (periodic acid-Schiff) stain of nail clippings — faster and more sensitive than culture; or PCR molecular testing — fastest and most sensitive but most expensive. In-office KOH preparation can give a rapid result but is less sensitive.
